Middleton Clinic is appreciative of the trust that you have shown us in allowing our practitioners to provide for your medical needs. As part of our commitment to our patients and community, we continuously evaluate how to best meet the evolving healthcare needs of those we serve.

After extensive thought and consideration, we have made the decision to move our care team and operations to West Valley Medical Group — Caldwell Clinic.

Beginning August 10, 2026, Middleton Clinic will be closed, and we will no longer see patients at 381 S Middleton Rd, Suite B, Middleton, ID 83644.

On that date, Christina Jones, DNP, and the Middleton Clinic care team will begin seeing patients at Caldwell Clinic, located at:

1906 Fairview Ave, STE 230
Caldwell, ID 83605

West Valley Medical Group will continue to be the custodian of your confidential medical records. You have the right to choose from whom to receive medical care. If you would like to receive a copy of your records or to transfer records to a new provider of choice, please complete the authorization for release of information form, sign, and return to:

Please bring your health insurance card to your initial visit, as well as subsequent visits.

If you are a new patient, please arrive 30 minutes before your scheduled appointment time to complete your paperwork. You may also download our patient registration forms in advance, fill them out, and bring them with you when you come for your appointment.
